Enrollment Forms- Elementary School
It is required by the State of Kansas to have on file and show proof of identification in the form of a certified copy of a state issued Birth Certificate and copy of a Social Security card in order to enroll in a State School.
Along with these items the State also requires that a first time student supply an up-to-date immunization record and a physical, completed by a physician or nurse practitioner.
School District USD 289 also requires other enrollment forms to be complete and on file prior to the first day of school.
