Great News! Our school has continued our partnership with Friendly Smiles Program to offer in-school
dental care. If your child does not see a dentist regularly or you would like them to see the in-school
dentist, simply fill out a consent form. New forms must be completed each school year.
Friendly Smiles accepts Medicaid, Commercial Insurance, and Uninsured. There is NO COST to you even
if your child is uninsured or not fully covered through commercial insurance. Uninsured students must
qualify for free and reduced lunch to participate in our program. Direct dental services provided to your
child while in school is completely FREE.
Easy & Convenient
Having your child seen by the in-school dentist saves you valuable time, as well as less time taken from
your child’s academics. Top quality care is provided by a licensed dentist and hygienist right in the
familiar surroundings of your child’s school. Dental care includes a complete exam, x-rays, cleaning,
fluoride, sealants, restorative care such as fillings, and when needed baby tooth extractions.

Simply fill out the attached Friendly Smiles consent form and return it to your school nurse today. You
may also access the form through our online secure forms by clicking the links below. The secure forms
